Job Summary
Clinical Transition Coordinator role supporting Mobile, AL with coordination of patient care transition from hospital/healthcare facility to alternative care site. Works with physicians, case managers, and internal teams to educate patients and families, secure referral sources, and coordinate care plans; acts as a nurse liaison between hospital providers and home-based infusion services; handles paperwork, payer communications, and collaboration with sales/territory management to drive referrals. Requires active Alabama RN license, 5+ years of clinical/home health experience, knowledge of discharge planning and care coordination, ability to educate providers and patients about services, and experience improving care processes.
Required Qualifications
- Active Alabama RN License, in good standing
- Minimum five (5) years of clinical experience, home health field or management experience preferred
- Knowledge of hospitals, skilled nursing/rehab facilities including case management and discharge planning service needs
- Advanced practice expertise to identify and implement improvement processes, and the ability to design, direct and implement health care plans on behalf of the patient
- Preferred experience partnering with sales teams to drive referrals
