Job Summary
Provide high-quality, evidence-based care to adults, young adults, and/or children within an integrated care model. Work in a collaborative clinical team with monthly learning opportunities and built-in referrals, while completing timely, accurate documentation and supporting patients in crisis with appropriate interventions. Required Qualifications
