Job Summary
Provide neurobehavioral health services to residents in long-term care facilities as a Licensed Professional Clinical Counselor (LPCC) or Licensed Independent Social Worker (LISW). Work within a multidisciplinary treatment team to deliver holistic care for elderly individuals with neurocognitive disorders and/or psychiatric conditions. Partner with specialty clinicians to coordinate care, utilize an EMR system for documentation, and contribute to productive, autonomous practice with support from an Area Manager. The role emphasizes gero-psychosocial care, dementia management, interdisciplinary collaboration, and adherence to clinical care protocols in a long-term care setting. Required Qualifications
- Master’s degree in Counseling or in Social Work
- Currently licensed as an LPCC or LISW in the state in which you are applying
