Job Summary
Oversee all patient care services by coordinating referrals, assessing needs, and developing individualized plans of care. Review and approve care plans for clinical appropriateness while conducting regular OASIS reviews and ensuring required visits are completed timely. Monitor documentation timeliness for direct care team members and LPN/LVNs to maintain compliance with State and Federal regulations. Participate in strategic planning and QAPI activities within a physically demanding, high-stress environment. Requires an Associate degree in Nursing, two years of home care experience, and ability to lift up to 50 pounds.
Required Qualifications
- Associate degree in a Nursing related field
- 2 years home care experience as a registered nurse in a Home Health or Hospice environment
- Current, unrestricted RN license valid for the state of work
- Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must be able to travel on occasion to support a field office in time of audit or necessitating event
- Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
