Job Summary
Clinical Team Manager overseeing patient care services and direct care staff in home health. Responsibilities include assigning patients and personnel, coordinating patient care and referrals, approving and updating the plan of care, conducting regular OASIS reviews, hiring and supervising direct care staff, and coordinating with physicians and family. Requires RN license, ability to travel within the branch/coverage area, proficiency with enterprise applications (e.g., Homecare Homebase, Workday) and Microsoft Office, and the capability to work in a physically demanding, high-stress environment. Required Qualifications
- Associate degree in Nursing related field
- 2 years home care experience as a registered nurse in a Home Health or Hospice environment
- 1 year of supervisory experience preferred
