Clinical Team Lead - LPN/RN

Provide clinical leadership and support to nurses, aides, therapists, and social workers while coordinating patient care, clinician assignments, and referrals. Oversee care plans to ensure quality outcomes and regulatory compliance, review clinical documentation, and support timely billing and reimbursement processes. Monitor high-risk patients, collaborate with clinicians to reduce hospitalizations, and conduct chart audits to participate in quality improvement initiatives. Serve as a clinical resource and mentor for field staff, supporting onboarding, education, and competency development. Step into leadership responsibilities in the absence of the Clinical Director/Manager. Requires an active Ohio RN or LPN license, minimum 3 years of clinical nursing experience, and at least 1 year of home health experience.
