Job Summary
Oversee daily clinical services to ensure timely patient care and appropriate utilization of resources while monitoring staffing coverage, visit completion, and workflow challenges. Partner with scheduling to maintain adequate staffing and optimize clinical productivity, leading daily team prioritization and staff performance efforts. Ensure consistent implementation of individualized care plans and adherence to standards across interdisciplinary teams by facilitating Interdisciplinary Team (IDT) meetings and conducting ride-alongs, shadowing, coaching sessions, and chart reviews. Support staff through coaching, feedback, and ongoing performance improvement initiatives, ensuring clinical services are available 24 hours per day, 7 days per week, including participation in a manager on-call rotation. Collaborate with physicians, the Medical Director, and interdisciplinary team members to coordinate patient care and support patient and family goals while building relationships with healthcare facilities and community stakeholders.
Required Qualifications
- Valid Michigan licensure as a Registered Nurse (RN)
- Minimum of 5 years of healthcare experience
Desired Qualifications
- BSN
- hospice experience
- Previous leadership, management or supervisory experience
- Proficiency with Electronic Medical Records (EMR) systems and Microsoft Office applications
