Job Summary
Clinical Support Specialist in Care Transitions monitors patients in a recuperative care environment, takes vital signs, observes patients, reports changes to nursing staff, coordinates medical appointments and refills, assists with medication management and documentation in the Electronic Health Record, and supports multidisciplinary teams while ensuring confidentiality and HIPAA compliance. This role requires compassionate care for underserved populations, ability to work with diverse groups, and readiness to work holidays and alternate shifts in a high-acuity setting.
Required Qualifications
- High School Diploma or equivalent required
- Medical Assistant or CNA training/certification highly preferred, OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
- Minimum of 1 year of patient care experience in a medical facility, substance use disorder treatment facility or behavioral health environment.
- Must have current BLS Certification or willingness to obtain within 90 days from date of hire.
