Clinical Support Coord - Mid Shift - LPN, MA

Obtain informed telephone authorization and conduct comprehensive medical and social history risk assessments with families of patients who authorize donation. Collaborate directly with clinicians to perform medical screenings on patients to determine donation eligibility. Manage all aspects of tissue and shared organ cases, ensuring seamless transitions and effective case completion while providing accurate, timely information to internal and external stakeholders. This rotating 12-hour shift schedule runs from 1:00 PM to 1:00 AM, including weekends and holidays, alternating between four-day and three-day workweeks. Candidates require post-secondary training in a relevant field with strong case management and communication skills.
