Clinical Supervisor - RPN

Job Summary
As Clinical Supervisor for RPNs, lead and support Personal Support Workers to ensure compliant, safe home-health care delivery; conduct regular huddles and one-on-ones; drive incident investigations and ensure timely action; collaborate with nursing to support clinical documentation and funder reporting; engage patients and staff in problem solving, mentoring, and health-teaching; maintain current knowledge of practice standards and evolving home health care practices; require strong leadership, organizational, and teaching skills to develop staff and ensure high-quality community nursing care.
Required Qualifications
- Current RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions
- Minimum of 5 years’ experience as a practicing Registered Practical Nurse
- Comprehensive patient health assessment skills
- Understanding of community nursing practice principles
- Proven experience leading and managing a team
- Knowledge of clinical documentation standards and practice guidelines
- Excellent organizational, interpersonal, and teaching skills
- Commitment to professional development in clinical practice relevant to home health care
- A valid driver’s license and access to reliable transportation
- Ability to provide a current, clear Vulnerable Sector Screening (VSS)
- C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset
