Clinical Supervisor- RPN

Job Summary
Supervise and support Personal Support Workers to ensure practices comply with legislated standards and company policies. Respond to and resolve concerns from community staff, patients, and families through regular huddles, face-to-face meetings, and one-on-ones. Initiate patient safety incident investigations and document action plans while collaborating with nursing teams on clinical documentation and funder reporting guidelines. Engage patients and staff in problem-solving, offering health teaching, coaching, and mentoring to identify care needs and build trust. Maintain current RPN registration and comprehensive patient assessment skills to provide safe, efficient service in home health care settings.
Required Qualifications
- Current RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions
- Minimum of 5 years' experience as a practicing Registered Practical Nurse
- Comprehensive patient health assessment skills
- An understanding of the principles of community nursing practice
- Proven experience leading and managing a team
- Knowledge of clinical documentation standards and practice guidelines
- Excellent organizational, interpersonal, and teaching skills
- Effective critical thinking and problem-solving skills
- A valid driver's license and access to reliable transportation
- Able to provide a current, clear Vulnerable Sector Screening (VSS)
Desired Qualifications
- C.N.A. Certification as a Community Health Nurse (CCHN(C))
