Job Summary
Receive case referrals, review patient information, and assign hospice personnel to determine home care needs. Confer with the Medical Director and Attending Physician regarding eligibility, then evaluate cases through home visits, conferences, and records review. Guide clinicians on impressions and procedures, conduct clinical record reviews, and communicate findings to the Clinical Director. Assist in screening new employees, orienting staff, and developing in-service education programs. Participate in quality assessment teams and community activities, while assisting in policy formulation and planning. Available Monday through Friday, 8 AM to 5 PM, covering Snohomish County.
Required Qualifications
- A Registered Nurse license in good standing to practice professional nursing in the state
- Must have previous experience in a hospice home care setting with two years of management or supervisory experience
- Understanding of the hospice philosophy and issues of death and dying
- Excellent observation, verbal, and written communication skills
- A valid driver's license
- Current CPR certification
