Clinical Supervisor: Physical Therapy

Clinical Supervisor - Therapy oversees field-based clinician operations and patient care coordination for home health services. Serves as first point of contact for field clinicians, supervises clinical personnel, and ensures care and services are delivered appropriately. Hybrid role with 2 days in the Walnut Creek office and 3 days in the field, including 12 patient visits per week for supervisory visits. Responsibilities include in-field oversight of clinicians, timely handling of calls, coordinating training and education programs for nurses, assessing staff learning needs and forming goals, facilitating bi-weekly Case Conferences, reviewing and guiding case services, establishing therapeutic goals and plans of care, and remaining available during operating hours. Requires a baccalaureate or master's degree in a related discipline, CA Physical Therapist license, BLS, and CA driver’s license. Must have at least two years of management or supervisory experience in rehabilitation therapy, preferably home care.
