Job Summary
Review overnight updates and priorities in Home Care Home Base, then align the team on schedule adjustments and patient needs. Conduct midday chart review to verify OASIS items, clarify documentation questions, and ensure records are audit-ready. Connect with clinicians and coordinators to remove barriers, provide guidance, and keep care plans moving forward. Travel regionally throughout Maricopa County to support field teams, standardize processes, and address time-sensitive issues. Finish by confirming key items are closed out in the system and setting up the next day's follow-ups.
Required Qualifications
- Home health care experience with advanced HCHB proficiency
- Working knowledge of HHRG optimization/utilization and OASIS correction processes
- Strong compliance focus, problem-solving, and attention to detail
- Effective communication and staff coaching capability
- Willingness to travel regionally
