Job Summary
Establish and evaluate day-to-day activities of the Care Management Department, overseeing Care Manager assignments, staffing levels, and time-off scheduling. Provide consistent feedback and recognition to staff while utilizing conflict resolution skills to ensure timely issue resolution. Identify, plan, and coordinate patient and family care aspects, initiating required forms and letters to comply with regulatory guidelines. Complete timely documentation and educate the interdisciplinary team on department goals. Collaborate with other departments to achieve positive patient outcomes. Requires current New York State R.N. license, BSN or related master's degree, and acute care management experience.
Required Qualifications
- current New York State R.N. License
- Graduate of an accredited Nursing School or College
- BSN Bachelor of Science in Nursing or Health
- Master Degree in related field
- Experience in acute care Care Management
- Basis knowledge of personal computer and software for word processing and spreadsheets and/or Hospital Information Systems
Desired Qualifications
- Approved National Certification
- Management experience in Care Management
