Job Summary
Receive case referrals and review patient information to determine hospice needs, then assign appropriate personnel and confer with the Medical Director regarding eligibility. Review and evaluate cases through home visits, conferences, and records, while discussing impressions with clinicians to guide effective home care delivery. Conduct clinical record reviews, communicate findings to the Clinical Director, and assist in screening new employees and orientation. Support the Director of Patient Care Services in planning continuing education programs and formulating organizational policies. Participate in quality assessment performance improvement teams and community activities promoting the organization's role in health care delivery.
Required Qualifications
- A Registered Nurse license in good standing to practice professional nursing in the state
- Must have previous experience in a hospice home care setting with two years of management or supervisory experience
- Understanding of the hospice philosophy and issues of death and dying
- Excellent observation, verbal, and written communication skills
- A valid driver's license
- Current CPR certification
