Job Summary
Receive case referrals, review patient information to determine hospice needs, and assign appropriate personnel. Confer with Medical Directors and Attending Physicians regarding eligibility, then evaluate cases through home visits, conferences, and records review. Discuss impressions with clinicians, instruct on effective performance, and conduct clinical record reviews to communicate findings to the Clinical Director. Assist in screening new employees, orienting hires, and planning continuing education programs. Formulate and evaluate organization policies, assess community health needs, and participate in quality assessment performance improvement teams. Comply with professional standards and perform other duties as delegated by the Director of Patient Care Services.
Required Qualifications
- A Registered Nurse license in good standing to practice professional nursing in the state
- Must have previous experience in a hospice home care setting with two years of management or supervisory experience
- Understanding of the hospice philosophy and issues of death and dying
- Excellent observation, verbal, and written communication skills
- A valid driver's license
- Current CPR certification
