Job Summary
Receive case referrals, review patient information to determine hospice needs, and assign appropriate personnel. Confer with the Medical Director and Attending Physician regarding eligibility, then review and evaluate cases through home visits, conferences, and records. Discuss impressions with clinicians to guide effective performance, conduct clinical record reviews, and communicate findings to the Clinical Director. Assist in screening new employees, orienting staff, and planning continuing education programs. Participate in quality assessment teams and community activities while assessing community health needs. Comply with professional standards and perform other duties as delegated by the Director of Patient Care Services.
Required Qualifications
- A Registered Nurse license in good standing to practice professional nursing in the state
- Must have previous experience in a hospice home care setting with two years of management or supervisory experience
- Understanding of the hospice philosophy and issues of death and dying
- Excellent observation, verbal, and written communication skills
- A valid driver's license
- Current CPR certification
