Job Summary
Receive case referrals and review patient information to determine hospice and home care needs, then assign appropriate personnel. Confer with the Medical Director and Attending Physician regarding eligibility, while reviewing and evaluating cases through home visits, conferences, and records analysis. Guide clinicians on impressions and clinical courses, conduct clinical record reviews, and communicate findings to the Clinical Director. Assist in screening new employees, orienting staff, and developing in-service education programs. Participate in quality assessment teams and community activities, while formulating and evaluating organizational policies and goals. Comply with professional standards and assist in planning patient care services.
Required Qualifications
- A Registered Nurse license in good standing to practice professional nursing in the state
- Must have previous experience in a hospice home care setting with two years of management or supervisory experience
- Understanding of the hospice philosophy and issues of death and dying
- Excellent observation, verbal, and written communication skills
- A valid driver's license
- Current CPR certification
