Job Summary
Oversee day-to-day operations of the subway outreach case management team (ILSs) and supervise team members; ensure engagement, assessment, placement, and follow-up for chronically homeless individuals; maintain up-to-date clinical charts including psychiatric evaluations, narrative psychosocials, and housing plans; evaluate clients for 9.58 removal; participate in clinical management meetings and case conferences; prepare monthly and other reports; provide CPR/First Aid or overdose prevention protocol as needed.
Required Qualifications
- LMSW
- ability to obtain 9.58 removal designation
- valid driver’s license
- CPR training certification or willingness to take CPR training
- basic computer literacy
