Job Summary
Clinical Staff Pharmacist provides safe, effective, and prompt medication therapy; performs drug preparation and distribution, input/verification of medication orders, and serves as a drug information resource. Delivers decentralized/pharmacy clinical services including anticoagulation, pharmacokinetic and renal dosing, nutrition support, and antimicrobial/opioid stewardship; provides patient counseling; collaborates with inter-professional teams to support transitions of care; participates in unit-based activities and maintains knowledge of regulatory requirements and inventory management.
Required Qualifications
- Graduate of an accredited college or university with Pharmacy degree
- Licensed Pharmacist with the Kentucky State Board of Pharmacy in good standing
- Pharm.D. or relevant clinical hospital pharmacy experience is desirable
