Job Summary
Pharmacist responsible for dispensing medication in response to physician prescriptions, counseling physicians/nurses/community on appropriate medication use, monitoring anticoagulation programs, and ensuring safe, effective drug therapy in an inpatient hospital setting. Requires PharmD (or equivalent) and state licensure in Michigan; ACLS for certain shifts; experience in anticoagulation management and oncology is preferred. Responsibilities include reviewing/dispensing prescriptions, counseling, arranging IV therapy, pharmacokinetic evaluation, medication safety, and collaboration with clinical teams to support patient care across acute care settings.
Required Qualifications
- Bachelor’s degree in Pharmacy or PharmD
- Licensed Pharmacist in good standing in Michigan
- ACLS certification for ED/MICU staff (if regularly scheduled) or equivalent training (preferred)
- Ability to dispense medications and provide patient/pharmacy staff education
- Experience with anticoagulation management (preferred)
- Oncology pharmacotherapy knowledge or training (preferred)
Desired Qualifications
- Oncology knowledge or PGY-2 training in oncology (preferred)
- PGY-1 training (preferred)
- ACLS certification for ED/Adult MICU staff (preferred)
- Experience with anticoagulation management and anticoagulation clinics
- Strong clinical knowledge and communication skills
- Experience with IV medications, pharmacokinetics, and drug utilization reviews
- Hospital pharmacy experience
- ACLS certification for pharmacists who staff ED or MICU (preferred)
