Job Summary
Provide clinical support services to APC S clients and case managers, including screening prospective Skilled Nursing clients and securing MD orders. Conduct chart audits, create care plans, and manage admission and discharge paperwork while participating in case conferences and the PI committee. Supervise clinical field staff, coordinate education sessions for CHHA, LPN, and RN staff, and mentor new team members. Complete incident forms, document staff and client complaints, and assist with private duty nursing cases. Ensure case manager visits are completed and comply with accrediting body standards. Provide weekend and evening RN call coverage as needed.
Required Qualifications
- Clinical Specialist experience
- Private duty nursing (PDN) case management
- Supervision of clinical field staff
- Case management to clients
- Compliance with policies and procedures for accrediting bodies and/or licensing agencies
- Documentation of staff/client complaints and concerns
- Development of in-service programs
- Conducting education sessions
