Job Summary
Clinical Nurse Specialist (Home Health) provides expert clinical guidance to nurses, therapists, and aides via phone/telehealth and in-person interactions; conducts telephonic and virtual patient assessments; manages care plans and documentation; leads quality assurance initiatives, regulatory compliance, and care coordination between providers, patients, and caregivers; develops and oversees initial and recertification Plans of Care; participates in interdisciplinary team meetings, mentors staff, and may perform in-person visits on a PRN basis; supports administrator-on-call rotation and telehealth/EMR-enabled patient care with a focus on continuous quality improvement.
Required Qualifications
- Active RN or LPN license in Washington State or Multistate RN/LPN license
