Job Summary
Coordinate services for recovery, employment, education, support groups, and housing within agency reintegration programs under intensive clinical supervision. Document individual progress, prepare discharge summaries, and provide case management to enhance daily living skills while meeting with residents regularly. Make referrals, coordinate services with external providers, and assist with admission processes and orientation. Ensure resident safety, respond to immediate social or housekeeping issues, and conduct breathalyzer and toxicology screenings as needed. Provide resident education, facilitate program guidelines, and serve on agency committees while maintaining compliance with federal and state regulations.
Required Qualifications
- Minimum of HS Diploma or GED
- Enrolled in 350-hour educational program or completion of one-time requirements within 3 months of hire
- At least one year of experience in the substance use, mental health, or human services field
- Must possess a valid New York State Driver's license and reliable automobile
- CASAC-Trainee or Provisional certification
- Completion of a 350-hour educational program or 4000 hours of work experience
- 85 hours of Section I education
- Completion of OASAS one-time requirements
- CASAC or other QHP status certification
