Job Summary
Provide social work services across adult and pediatric settings with a focus on holistic, patient-centered care for vulnerable populations. Conduct comprehensive evaluations of psychosocial and mental health status, manage legal and safety considerations, and lead crisis intervention and transition planning. Coordinate internal and external services, implement behavioral health and addiction intervention strategies, and facilitate end-of-life care discussions with surrogates, families, and providers. Deliver brief counseling, lead support or treatment groups, and enhance patient capacity to address their own needs through collaborative care planning.
Required Qualifications
- Master of Social Work degree from an accredited School of Social Work
- Minimum of one year relevant experience
- Licensed Clinical Social Worker (LCSW) for employees who perform this job in the state of Oregon
- Licensed Independent Clinical Social Worker (LICSW), Advanced Social Worker (LASW), or Licensed Social Work Associate & Advanced (LSWAA) in the State of Washington
