Job Summary
Conduct thorough psychosocial assessments of patients in relation to their illness and environment, then communicate findings to the multidisciplinary home health team. Carry out social evaluations to develop effective intervention plans that enhance patient experience and care outcomes. Maintain accurate clinical records, provide referral services for practical and environmental needs, and act as a liaison between patients and community agencies. Work closely with the interdisciplinary team to develop comprehensive care plans, participate in case conferences, and assist in discharge planning. This per diem role serves patients in the Manchester area within Pathways Healthcare's Medical Rehabilitation Program.
Required Qualifications
- Holds a master's degree from a school of social work accredited by the Council on Social Work Education
- Holds a valid license as an LICSW in good standing
- Possesses a minimum of one (1) years' experience in health care
- Demonstrates excellent verbal and written communication skills
- Maintain current BLS CPR certification
- Possess a valid driver's license
- Own a reliable, insured vehicle
