Clinical Social Worker, Outpatient Endocrinology

Provide clinical social work services to children, adolescents, and families in the Endocrine outpatient program, with a focus on diabetes patients. Perform psychosocial and risk assessments, assign diagnoses, and develop treatment plans. Participate in planning and implementation of discharge and outpatient service plans while collaborating with the health care team and community agencies to follow up on needed services. This full-time role requires LICSW licensure in Massachusetts and is based in Boston with Weymouth coverage as needed. New hires receive a $5,000 sign-on bonus.
