Job Summary
Medical Social Worker (MSW) or LCSW for Home Health in San Jose, CA (per diem). Provide in-home social work services, education and support to patients and families, and coordinate with an interdisciplinary team to integrate care. Assess social and emotional factors related to illness/injury, develop and implement plans for home social work services, and educate patients, families, and caregivers. Requires knowledge of local community resources, understanding of health care regulations for home care, strong written and verbal communication, ability to work with diverse populations, time management, and professional licensure maintenance. Masters degree required; BLS and Driver License required; LCSW license is highly preferred; Home Health experience and one year of MSW experience in healthcare setting strongly preferred. Office is in San Jose with field home visits; travel within primary territory may occur.
Required Qualifications
- Masters Degree from a CSWE-accredited program
- Basic Life Support Certification (BLS)
- Driver License
- Must maintain licensure and certifications
- One year of experience as an MSW in a health care setting strongly preferred
- Home Health experience preferred
- LCSW license is highly preferred (supervision provided to license-eligible Social Workers)
