Job Summary
Medical Social Worker (MSW/LCSW) for in-home home health visits in the San Francisco Bay Area. Provides education and support to patients and families, develops and implements home social work plans, coordinates with interdisciplinary team and community resources, and ensures regulatory compliance. Requires a Master’s degree in Social Work, active or eligible LCSW, BLS certification, and a valid driver license with automobile insurance. Per diem schedule with ~15-20+ home visits weekly; field-based role focused on geriatric patients and in-home care planning. Demonstrated knowledge of local resources, regulatory guidelines, exceptional communication, and ability to manage diverse, sensitive situations; supervision available for license-eligible professionals.
Required Qualifications
- Masters Degree from a CSWE-accredited program
- LCSW license or eligibility
- Basic Life Support (BLS) certification
- Driver license
- Automobile Insurance
- One year of MSW experience in a health care setting (strongly preferred)
- Home Health experience preferred
- Knowledge of local community resources
- Knowledge of state and federal homecare regulatory guidelines is preferred
- Effective written and verbal communication skills
- Ability to manage time and prioritize
- Ability to engage diverse populations and provide culturally competent care
- Maintains all required licensure and certifications
- Orientation and annual core competencies for position
- Supervision provided for license-eligible Social Workers (LCSW)
