Clinical Social Worker MSW

Collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs, including alternate site coordination, discharge planning, and addressing abuse or neglect. Provide referrals for substance use disorders, facilitate guardianships, and offer bereavement support, adjustment counseling, and behavioral health concerns such as psychiatric transfers. Conduct individual and family assessments, perform crisis interventions, and address grief and loss counseling. Maintain a Master's degree in social work with two years of relevant experience, including health care and care management expertise. Work part-time, 26 hours weekly, Monday through Friday at the Care Center Hubbard Dr in Dearborn.
