Job Summary
Conduct comprehensive evaluations of psychosocial and mental health status for adult and pediatric patients across the health care continuum. Provide crisis intervention, transition planning, and coordination of internal and external services while advocating for patients, surrogates, and families. Deliver brief counseling, substance use support, and end-of-life care strategies using a strengths-based approach within an interdisciplinary team. Manage legal and safety considerations and identify community resources to address social determinants of health.
Required Qualifications
- Master of Social Work degree from an accredited School of Social Work
- Clinical Social Work Associate (CSWA, working toward LCSW) license in OR
- Licensed Social Worker (LSWAIC, working toward LICSW) license in WA
- American Heart Association (AHA) issued BLS Provider certification
- Licensed Clinical Social Worker (LCSW) license in OR
- Licensed Independent Clinical Social Worker (LICSW) license in WA
Desired Qualifications
- 1 year of relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization
- 3 years' relevant experience
Additional Requirements
- Associates in WA may begin work with a pending associate license and may work for a maximum of 120 days while awaiting licensure approval; if approval is not obtained within 120 days, the associate would be required to cease work immediately.
