Job Summary
Conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. Advocate for clients and collaborate with multidisciplinary teams to ensure holistic support. Complete and submit documentation following established guidelines, review clinical progress toward goals, and document follow-up of identified problems. Write physician orders according to policy. Requires current California LCSW license, 2 years in health care, and Basic Life Support certification. Located at Dignity Health - Mercy San Juan Medical Center in Carmichael, California.
