Job Summary
Provide social work services during the 8:00 AM – 4:30 PM day shift in the ICU, conducting thorough psychosocial assessments for patients and families in crisis. Develop individualized treatment and discharge plans that address emotional, social, and practical needs while delivering brief counseling and crisis intervention. Coordinate with community agencies and internal resources to support safe care transitions, serving as a patient advocate and liaison between care teams and families. Document assessments, plans, interventions, and outcomes in a timely and accurate manner while collaborating with the interdisciplinary care team to support holistic patient care.
Required Qualifications
- Master's Degree in Social Work or a related field from an accredited institution
- Eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work
- Minimum of one year of social work experience in a healthcare setting
- Exceptional crisis intervention and psychosocial assessment skills
- Strong emotional resilience and sound decision-making under pressure
- Compassionate communication and active listening
- Collaborative team player who thrives in a dynamic, high-acuity environment
- Familiarity with local resources and referral processes for at-risk populations
- Accurate, timely clinical documentation and case tracking
