Required Qualifications
- Master's degree in Social Work
- Completed CMS’s Long-Term Care Basic Training
- Passed the Surveyor Minimum Qualifications Test (SMQT)
- At least two (2) years of recent experience as a surveyor
- Current licensure to practice as an LCSW
- U.S. Citizenship
- Ability to travel up to 75% of the time
