Job Summary
Conduct comprehensive psychosocial and mental health evaluations for adult and pediatric patients across the health care continuum. Provide crisis intervention, transition planning, and coordination of internal and external services while advocating for patients, families, and surrogates. Implement strengths-based behavioral health interventions for individuals, families, or groups addressing substance use, chronic illness, and end-of-life care. Collaborate within an interdisciplinary team to triage needs and identify community resources. Maintain accurate documentation and navigate electronic medical records in compliance with legal and regulatory requirements.
Required Qualifications
- Master of Social Work degree from an accredited School of Social Work
- Clinical Social Work Associate (CSWA, working toward LCSW) license in OR
- American Heart Association (AHA) issued BLS Provider certification
- Licensed Clinical Social Worker (LCSW) license in OR
Desired Qualifications
- 1 year of relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization
- 3 years' relevant experience
