Clinical Services Director Home Health

Home Health Director Clinical Services (RN) oversees patient and family care per the plan of care, directs the Interdisciplinary Team (IDT) including scheduling, mentoring and monitoring, and serves as the liaison between the IDT and Primary Care Physician. Responsibilities include ensuring regulatory compliance, leading IDT meetings, and managing EMR documentation workflow. Requires RN licensure in the state of employment and at least one year of home health care experience with supervisory experience preferred. Strong emphasis on culture of inclusion and providing high-quality, compassionate home-based care.
