Job Summary
Coordinate patient care as a SHC coordinator, collaborating with health team members to provide required treatment. Act as a patient teacher and advocate by assisting patients in seeking information, ensuring informed consent for treatment decisions, and promoting patient-desired independence. Deliver individualized care that addresses needs holistically, enabling patients to live rather than merely survive dialysis. Maintain competency in Electronic Health Records and organize projects independently while serving as a servant leader within a multi-disciplinary team.
Required Qualifications
- Graduation from an accredited nursing school or equivalent
- Current, active, unencumbered state RN licensure
- Current Healthcare Provider certification (CPR)
- Competent and in-depth knowledge regarding the use of Electronic Health Records
- Ability to organize, prioritize and complete projects independently
