Job Summary
Coordinate patient care as a SHC coordinator, collaborating with care providers to deliver individualized treatment. Act as a patient teacher and advocate by assisting patients in seeking information, ensuring informed consent for treatment decisions, and promoting maximum patient-desired independence. Work directly with a multi-disciplinary team to address patient needs holistically, enabling patients to live rather than just endure dialysis. Maintain current, active, unencumbered state RN licensure and CPR certification while utilizing in-depth Electronic Health Records knowledge. Required Qualifications
- Graduation from an accredited nursing school or equivalent
- Current, active, unencumbered state RN licensure
- Current Healthcare Provider certification (CPR)
- Competent and in-depth knowledge regarding the use of Electronic Health Records
- Ability to organize, prioritize and complete projects independently
