Job Summary

Coordinate clinical activities by delegating tasks to Licensed Practical Nurses, Medical Assistants, and Scheduling Clerks while triaging and assessing participants in the clinic setting. Supervise staff to ensure competency, collaborate with physicians, nurse practitioners, and interdisciplinary team members on care delivery and transitions, and document all clinical interventions in participant medical records. Provide education resources for acute and chronic illnesses, focusing on enhancing functional capacity and encouraging autonomy for participants, caregivers, and families. Ensure adherence to quality initiatives and evidence-based standards while managing care for frail elderly populations. Drive between locations as needed to support daily clinical operations.

Required Qualifications

  • Must have the ability to lead and direct staff in the delivery of care
  • Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact
  • Excellent oral and written communication skills
  • Must have strong interpersonal skills
  • Must have the ability to understand and standardize process related to quality indicators and utilize technology to document and present data
  • Must be flexible, with imagination and group leadership skills
  • Position requires the ability to drive between locations on a daily basis as needed

Desired Qualifications

  • BSN preferred
  • At least one year working with the frail elderly population
  • Five years of management experience preferred
