Job Summary

Conduct health assessments and examinations for participants enrolled in the Institute on Aging PACE program. Serve as a member of the Intake and Assessment Team to evaluate needs and develop comprehensive nursing treatment plans. Provide and coordinate nursing care in participants' homes and at the Day Health Center, managing minor illnesses, injuries, and stable chronic conditions. Initiate preventive and rehabilitative procedures, evaluate acute illnesses with primary care providers, and coordinate referrals. Teach and counsel participants and families on care needs while consulting with home care staff. Review charts to ensure medical plans are carried out and conduct home visits as directed. Provide coverage in the Primary Care Clinic Monday through Friday with Saturday work on a rotating basis.

Required Qualifications

  • Current California Registered Nurse license
  • At least two years of nursing experience at the Registered Nurse level
  • Ability to work effectively as part of an interdisciplinary team
  • Demonstrated initiative, imagination, resourcefulness, and flexibility
  • Interest in and willingness to work in a multilingual and multicultural environment

Desired Qualifications

  • Public Health Nurse Certificate
  • Experience in public health nursing, acute care nursing, or working with older adults
  • Computer literacy
  • Fluency in Spanish, Russian, or Chinese
