Job Summary
Conduct comprehensive nutritional assessments for new admissions and quarterly reviews, developing and monitoring individualized nutrition care plans. Provide nutrition education and counseling to residents and families while collaborating with physicians, nurses, and therapists on interdisciplinary care. Ensure compliance with CMS and DOH regulations by maintaining accurate clinical documentation and performing weekly weight monitoring to identify hydration concerns. Participate in quality improvement initiatives, conduct monthly kitchen sanitation audits, and review facility menus semi-annually. This full-time hybrid role offers 40 hours per week with primarily remote work and one on-site visit per month. Salary ranges from $66,560 to $74,880, including travel time and mileage reimbursement.
Required Qualifications
- Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program
- Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR)
- Current state licensure/certification (if required by the state)
- Minimum of one year of clinical nutrition experience preferred
- Excellent communication and interpersonal skills
- Strong critical thinking and clinical decision-making abilities
- Experience developing and implementing individualized nutrition interventions
- Familiarity with state survey processes and long-term care nutrition regulations
- Competency with electronic medical record (EMR) systems
- One required on-site visit per month
