Job Summary
Ambulatory Clinical Pharmacy Specialist at Shields Health Solutions. On-site role in a clinic at Providence Regional Medical Center Everett (Everett, WA) delivering patient-centered medication management under a Collaborative Practice Agreement (CPA). Build relationships with patients and providers, order and monitor evidence-based treatment plans, optimize therapy, resolve medication problems, support quality-improvement and REMS initiatives, and assist with complex prior authorizations while engaging across care teams.
Required Qualifications
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in Washington
- Completion of a PGY2 Ambulatory Care Residency
- Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
- At least 3 years of ambulatory practice with disease state management
- Strong communication, organization, and tech skills
- Empathy, integrity, and a team-first mindset
