Clinical Pharmacy Specialist - Hematology

Job Summary
Provide expert consultation and guidance on hematology medication use for cellular therapy, Car-T, lymphoma, and non-leukemia patients. Participate in multidisciplinary care planning meetings to optimize patient outcomes through evidence-based drug therapies and monitor for adverse reactions, medication errors, and drug interactions. Deliver pharmacokinetic and pharmacogenomic consultations, perform patient and caregiver education, and lead interdisciplinary committee work on protocol development and treatment plans. Maintain awareness of evolving clinical standards and support research activities as requested.
Required Qualifications
- Expert level of consultation, guidance, and direction for appropriate hematology affiliated medication use
- Complete care to the patient through consultative services and active participation in multidisciplinary patient care planning meetings
- Evidence-based, patient-centered drug therapies
- Monitoring for drug-related problems and medication cost containment
- Support for regulatory requirements and local quality programs related to safe medication use
- Systems to promote medication stewardship and surveillance
- Pharmacokinetic and/or pharmacogenomic consultations
- Detection, monitoring, documentation, and reporting of adverse drug reactions and medication errors
- Monitoring of drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
- Communication of ongoing patient information and efficacy and safety of treatment regimen(s) to all care providers
- In-service education to the hematology-oncology team and clinic staff
- Patient and caregiver education
- Counseling of patients regarding medication therapy and compliance
- Leadership and participation in interdisciplinary institutional committees
- Protocol and treatment plan development
- Education of pharmacists, nurses, physicians, and other providers
- Coordination of current patient issues and therapy recommendations with pharmacy staff
- Support of research activities of the respective specialty team
- Active participation in all respective areas of specialty team meetings
- Transitional care management
- Scientific research
- Educational teaching conferences
- Keeping abreast of changing protocols and clinical pharmacy standards
