Job Summary
Pharmacy leadership role overseeing design and management of clinical pharmacy programs including formulary management, policy development, education, and clinical consultation. Responsible for drug utilization reviews, adverse drug reaction reporting, medication-education initiatives, mentoring staff, and participating in interdisciplinary committees. Requires Doctor of Pharmacy or equivalent, extensive hospital pharmacy experience, completed or ongoing PGY2 residency, and active licensure with state boards; relocation available.
Required Qualifications
- B.S. or Pharm D from an ACPE-accredited School of Pharmacy required
- Doctor of Pharmacy from an ACPE-accredited college of pharmacy preferred
- Minimum of 1 year as a Pharmacy Clinical/Operations Specialist
- At least 5 years of hospital pharmacy experience
- Completion of hospital PGY2/pharmacy specialty residency or fellowship Required
- Specialty BCPS certification, ACLS, or other certification preferred
- Active Pharmacist license in good standing with the State Board of Pharmacy
- Active Pharmacy Consultant license required, or equivalent per State requirements if facility lacks Pharmacist in Charge
- Experience with Microsoft Office, Meditech, Business Objects and purchasing vendor applications
