Job Summary

Conduct virtual patient visits and in-person consultations to manage medication therapy, verify prescriptions, and counsel patients on dosage and side effects. Perform medication reviews, reconcile regimens, and assess adherence while monitoring for adverse reactions and drug interactions. Collaborate with interdisciplinary care teams to develop comprehensive care plans for high-risk patients, coordinate transitions, and optimize therapeutic outcomes to reduce rehospitalizations. Maintain accurate EHR documentation, generate quality improvement reports, and identify cost-saving opportunities through evidence-based prescribing. Full-time salary-based role with monthly bonus, 8-hour shifts Monday through Friday, and benefits including employer-matched 401K and HSA.

Required Qualifications

  • Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program
  • Licensed Pharmacist in good standing with the state board of pharmacy
  • Must be able to work flexible hours
  • Must be a licensed driver
  • Must have an automobile that is insured in accordance with state and/or organizational requirements
  • Must have an automobile that is in good working order
  • Ability to travel between offices, facilities, etc.
