Clinical Pharmacist

Job Summary
Clinical Pharmacist with responsibility for applying pharmacotherapeutic principles and disease-state/population management to deliver safe, appropriate, and cost-efficient pharmaceutical care. Develop and present new drug policies for the P&T Committee; collaborate with Health Plan physicians, pharmacists, nurses, and staff to perform utilization review of drugs and educate on policies; review literature and guidelines to support policy development; participate in CM/UM case conferences; provide clinical support for pharmacy benefits; conduct comprehensive MTM and medication reviews; work within interdisciplinary care teams to close gaps in care, reduce readmissions, and improve outcomes; ensure compliance with NCQA and CMS standards; update knowledge base on federal/state requirements; lead or contribute to policy writing and workflow improvements; support after-hours decision-making and respond to questions from members and providers.
Required Qualifications
- Doctorate of Pharmacy or Bachelor of Pharmacy with active and unrestricted license as a registered pharmacist in the U.S.
- Board of Pharmacy licensure in good standing
- Ability to complete accurate documentation of utilization management decisions, medication reconciliations and medication reviews
- Strong communication, problem solving, critical thinking, judgment and time management skills
- Ability to work independently and carry out assignments to completion without direct supervision
- Flexibility, ability to multi-task in a fast-paced environment, adapt to changing processes and priorities
- Ability to work as a team member
- Proficiency with Microsoft Office (word processing and spreadsheets) and the ability to navigate multiple systems
