Clinical Pharmacist - Surgery Relief

Job Summary

Conduct complex therapeutic problem resolution and independently interpret drug information for physicians, nurses, and patients. Monitor therapeutic responses and adverse effects, reporting all drug defects and errors while directing student and resident performance. Verify medication accuracy and supervise pharmacy technicians, assisting in discharge counseling and medication reconciliation from admission to discharge. Form peer-level relationships with attending medical staff to influence standards of therapy and develop treatment guidelines. Relief employees must work at least four days per pay period as determined by department needs. This role integrates medication use systems with centralized functions within a flexible decentralized care model at Oregon's only public academic health center.

Required Qualifications

  • A current Pharmacist's license issued by the Oregon State Board of Pharmacy
  • Relief employees must be available to work at least four (4) days per pay period

Desired Qualifications

  • BS in Pharmacy or Pharm.D.
  • Practice Residency or Specialist Certification (BCPS, BCCCP, BCOPS, etc)
  • PGY-2 Specialty Residency in Critical Care or Emergency Medicine
  • Vaccination certification
  • Emergency Department experience
  • Critical care experience
  • Familiarity with Code Blue Response
  • Outpatient pharmacy clerical rotation
  • Transitions of Care
  • Discharge Medication
  • Education
  • Surgical or Internal Medicine Services
  • Pain Management
  • Medication Reconciliation
