Job Summary
Conduct daily interdisciplinary rounds and manage specific medication therapies including warfarin, aminoglycosides, and vancomycin for high-acuity step-down ICU patients. Evaluate drug efficacy, monitor therapeutic goals, and review for contraindications while ordering laboratory tests per approved protocols. Order and evaluate lab tests to support medication therapy monitoring, identify opportunities for counseling during chart review, and communicate recommendations directly with providers. Electronically document interventions, provide sign outs, and fulfill provider-initiated pharmacy consults. Assist with projects such as MUEs, drug monographs, and performance improvement activities while reviewing and interpreting provider orders. Coordinate and supervise workplace activities, including satellite pharmacy operations and sterile product processing in the IV Admixture Service lab. Participate as a preceptor for pharmacy students and residents. Full-time 40-hour schedule with day shifts, evening components, and weekend/holiday coverage requirements.
Required Qualifications
- Bachelor of Science degree in Pharmacy
- PharmD degree from an accredited College of Pharmacy
- Current licensure in the State of Delaware
- Eligible to reciprocate licensure in the State of Delaware
Desired Qualifications
- ASHP-accredited pharmacy residency
- 3 years acute care experience
- Board certification
- English major
