Job Summary
Provide professional infectious diseases pharmaceutical care by monitoring select patients' medication therapy, reviewing medication profiles, laboratory results, and clinical parameters. Consolidate the antimicrobial formulary, ensure cost stewardship, and improve quality metrics related to antimicrobial use through collaboration with clinical leaders, physicians, and infection preventionists. Lead pharmacy clinical activities, support staff development to meet competency standards, and promote safe, appropriate, and cost-effective medication therapy while decreasing antimicrobial resistance and adverse reactions.
Required Qualifications
- Bachelor's Degree Pharm.D. or Bachelor degree in pharmacy
- 3 years of relevant experience
- 3 years of experience as a clinical pharmacist or pharmacy residency/fellowship
- LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
- LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
- MMR vaccine
- Varicella vaccine
- Tdap vaccine
- Influenza vaccine
Desired Qualifications
- Preferred residency training
- Preferred LIC-Clinical Pharmacist - STATE_MI State of Michigan
- Preferred LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan
